The increasing complexity of pharmacotherapy and the risks associated with inappropriate medication use have highlighted the need for strategies that promote patient safety. In this context, the present study aimed to analyze the impact of the clinical pharmacist’s role and pharmacovigilance, combined with health education, on the safe use of medications. This is an integrative literature review, conducted using the Latindex, SciELO, and Miguilim databases, including studies published between 2022 and 2026 in Portuguese and English. The results showed that the clinical pharmacist’s performance significantly contributes to the prevention of medication errors, reduction of adverse events, and improvement of therapeutic adherence. Pharmacovigilance stood out as an essential tool for monitoring adverse drug reactions and supporting safer clinical decision-making. Additionally, health education proved fundamental in raising patient awareness regarding rational medication use, reducing practices such as self-medication. It was also observed that the integration of these strategies enhances patient safety across different healthcare settings. It is concluded that strengthening the pharmacist’s role, combined with educational actions and effective pharmacovigilance systems, is essential to promote the safe use of medications and improve healthcare quality.
