Leprosy is a chronic infectious disease caused by Mycobacterium leprae and still represents a significant public health problem in Brazil, especially in regions with greater socioeconomic vulnerability. In this context, Primary Health Care plays a fundamental role in early identification, clinical diagnosis, and operational classification of the disease, contributing to the reduction of transmission and physical disabilities resulting from late diagnosis. This study aimed to analyze the management of leprosy in primary care, with emphasis on clinical diagnosis and operational classification of cases. This is a descriptive literature review, carried out from the analysis of scientific publications and official documents related to the topic. The results show that early diagnosis, based on the clinical evaluation of skin lesions and neurological changes, associated with the correct operational classification as paucibacillary or multibacillary, is essential for defining the appropriate treatment and for controlling the disease. Furthermore, the importance of the role of health professionals, especially the nursing team, in surveillance, patient follow-up, and health education of the population is highlighted. It is concluded that strengthening actions in primary care, coupled with the training of professionals and the expansion of access to health services, is fundamental for tackling leprosy and improving epidemiological indicators in the country.
