BETWEEN KNOWLEDGE AND PRACTICES: PATHS TO BUILDING SHARED CARE IN MULTIPROFESSIONAL HEALTHCARE PRACTICE

The increasing complexity of health systems has highlighted the need to overcome fragmented care models, strengthening practices based on the integration of knowledge and multiprofessional collaboration. In this context, shared care emerges as a central strategy for reorganizing healthcare work by promoting collective construction of care practices and shared responsibility among professionals. This chapter aimed to discuss the pathways for building shared care within multiprofessional healthcare practice, based on a theoretical analysis grounded in recent scientific literature. This is a qualitative study developed through a narrative literature review, with searches conducted in national and international databases. The findings indicate that interprofessional collaboration, effective communication, and patient-centered care are key elements for consolidating shared care practices. Organizational, cultural, and educational challenges that hinder the full implementation of multiprofessional work in everyday practice were also identified. It is concluded that building shared care requires changes in work processes, investment in interprofessional education, and strengthening collaborative practices in order to promote more integrated, humanized, and effective healthcare across different care settings.
